New Medication Profile Process (effective immediately)

Check the patient shipment for a printed copy of the Nufactor Medication Profile. This will be sent by the pharmacist with every shipment. It may be multiple pages. Note the Date Printed at the bottom of the profile.

 

    1. At the top of the Nursing Note Medication Profile, document the date at the bottom of the printed Nufactor Medication Profile that is in the shipment.

 

 

    1. Use the Nufactor Medication Profile to reconcile current medications. Ask the patient about all medications (including OTC’s, eye drops, topicals, herbals/supplements, home remedies, “as needed” medications) and compare to the printed copy.
    2. If ALL medications are the same, check the “No changes” box. No further action is needed.
    3. If a medication is added or changed:
      1. Check the “Medication added as below” box.
      2. Enter the medication information on the Medication Profile, check the “N” box (for new) and enter the date started.

 

    1. If a medication is discontinued:
      1. Check the “Medications discontinued as below” box.
      2. Enter the medication information on the Medication Profile, check the “D” box (for discontinued) and enter the date stopped.

 

If there is no Nufactor Medication Profile in the shipment, complete the entire Nursing Note Medication.
